Transfer of Medical Records

If you would like your medical records transferred to another doctor, we require a transfer of records form completed and sent to us from your new Doctor’s practice. Our policy is that a summary of your record is forwarded. If full records are required a fee of $27.50 will need to be paid by you as the patient, and a full copy of the file will then be sent via Disc or USB to the clinic of choice. The file will be either in ‘xml’ or ‘html’ format and will need to be Best Practice compatible. Otherwise, a paper record can be organised and mailed out. We hold the original records for medico-legal reasons.
