New Patient Information

All new patients first appointment will be charged an appointment fee at the full rate. This includes children under 16, Pensioners and Heath Care Card holders.

After your first visit, most GPs at this clinic will then bulk bill patients with concession cards and children under 16. Please talk to reception at time of booking.

If you are a new patient, you can save time by printing and completing the New Patient Form below to bring along to your first appointment. If you have made your booking online through HotDoc, you will receive a request from HotDoc with a link to complete your new patient form online for your convenience.
